Take an active role in your health. Complete a personal health assessment and schedule annual wellness exams to make a commitment for positive change. A simple health status questionnaire and conversations with your provider(s) helps you understand your health status and risk factors. Then, you can prioritize and take action before an illness or a chronic condition gets the better of you.


Map your plan to wellness

Be empowered to make lasting changes. Based on the results of your health assessment and annual wellness exam, you can create your own personal roadmap to wellness. Carle Health Wellness, supplemented by your medical care team, will support and guide you along your journey with resources, education, assessments and challenges to help you reach your personal goals. Understand your health

Gain a new perspective on your health and wellness. Health Alliance members can sign up for a free telephonic wellness consult with a health coach. This call is designed to help you better understand your overall health through a wellness lens. The consult can help you better understand areas of opportunity related to:

  • Nutrition
  • Weight loss
  • Physical activity
  • Management of chronic diseases like diabetes and hypertension

During the call the health coach also will make sure you understand your wellness benefits. This will include a review of recommended wellness screenings and information about additional free support available through the health plan.
